Marvel’s Spider-Man artist and former Insomniac character artist explains how he crafted Peter Parker’s nipples and bulge in an interview.

I settled on a bulge that was slightly larger than what’s visible on the [default suit]. The logic there being that he didn’t have as much constriction or support, so things would be a little more free to move around.
